CAPACITY BUILDING, HEALTHY COMMUNITIES and FIELD OF INTEREST GRANTS
All organizations receiving funding from Field of Interest Funds, which includes Capacity Building and Healthy Communities, are required to submit a comprehensive grant follow up report within one year of funding. If a multi-year grant has been made, a year one interim report is required, with a final report due at the close of funding. Upon completion of the project, grantee's final report will be subject to a rigorous review of programmatic and financial outcomes: progress toward achieving measures of success, efforts toward sustainability of the project in future years; and a description of the impact of the project on the organization. Please speak directly to Phil Koch, Executive Director, to determine specific requirements.
GREENSBURG FOUNDATION FUND GRANTS
Organizations receiving funding via Greensburg Foundation Fund are required to submit a final grant follow up report by December 1 of each calendar year if the project or funding opportunity has ended. Organizations will only remain eligible year to year if a follow up report is submitted. Please use the follow link to review the suggested format for that report.
DONOR ADVISED GRANTS
Not all donor advised grants require a follow up report. Certain donors may request that an organization follows up with details of how funds have been utilized, but, primarily only awards over $25,000 require a follow up report. You may utilize the grant follow up report link (below) for suggested formatting.
All follow up reports can be submitted by email to maximize efficiency. Follow-up Reports should be emailed to Caleb Crousey, Development and Program Officer at email@example.com
