The Community Foundation of Westmoreland County is an affiliate of The Pittsburgh Foundation, and all online applications and grant reports are processed through The Pittsburgh Foundation’s online grant portal here.

- Special Initiatives & General Grants: To submit a report for a general grant or a grant received from one of our special initiatives – Capacity Building, Healthy Communities, Revitalizing Westmoreland — download the Special Initiatives Grant Follow-Up Report Form.
- Greensburg Foundation Fund: To submit a report for a grant received from our Greensburg Foundation Fund, download the GFF Grant Follow-Up Report Form.
- Donor-Advised Grants: Not all donor-advised grants require a follow up report. Certain donors may request that an organization provide follow up on how funds have been utilized, but primarily only awards of more than $25,000 require a report. To submit a follow up report, download the Donor-Advised Follow-Up Report Form.
